We strive to accommodate you with timely appointments. Please help us by being on time for yours.


Your appointment time has been reserved and we’ve blocked out time to accommodate your oral care needs. If you must reschedule for any reason, please provide us with at least 48 hours notice so we can be considerate to other patients wishing to receive care. A cancellation fee may apply without proper notice.

Insurance Information 

Periodontics and Implant Dentistry accepts most major PPO dental insurance plans, including:


• Alliance
• Assurant
• Cigna
• Connection Dental
• Delta Dental
• Guardian
• Humana Dental
• Maverest


**Insurance only through Connection Dental:

• Aetna
• Ameritas
• MetLife PDP+
• United HealthCare
• United Concordia


*Please note:  Certain dental plans are only accepted within the umbrella of another plan.  We recommend calling to check if we are providers on your dental insurance. We are happy to work with your PPO dental insurance out-of-network, even if we are not a contracted provider.


If You Have Dental Insurance: As a courtesy, we will bill your insurance company for your visit to our practice, provided you furnish our office with a completed insurance claim form or insurance card. You may be responsible for a deductible and the percentage that your company requires at this visit. If we are not a contracted in-network provider for your insurance, we will complimentarily submit your dental claim but may expect payment in full at the time services are provided. This will vary depending on your specific insurance carrier.


If you are having future treatment, we would appreciate you paying this bill in full and applying your insurance payment toward the estimated portion of your future visits.


If You Don’t Have Dental Insurance: It’s customary for you to pay our office in full at the time services are rendered.

Financing and Payment Options 

The Periodontics and Implant Dentistry staff would like you to know that we’re committed to providing you with the best care possible. In order to achieve this goal, we need your assistance and understanding of our financial policy in the following areas:


• We offer a variety of fee arrangements to ensure that your treatment is affordable.

• We accept cash, checks, and most major credit cards (MasterCard, Visa, Discover and American Express).

• We offer our patients financing options through CareCredit.


We will be glad to discuss your options with you upon your arrival, and if you wish, our front office staff will discuss them with you. We realize that, from time to time, temporary financial setbacks may affect timely payments on your account. If such problems do arise, we encourage you to contact our office for assistance by calling 210-341-1489 to ask us any financial questions.
